Help For Seniors

In this section you will find a variety of services for seniors including social opportunities, crisis services and outreach clinics that can offer assessments by multidisciplinary teams. These clinics can assist in cases of seniors who are home-bound with mental health and dementia related conditions. Some clinics offer long-term follow up while others offer one time or short term intervention.

Crisis & Outreach Service for Seniors (COSS)

COSS is a mobile crisis intervention and outreach service for seniors that provides short-term response 365 days a year, 7 days a week, 9am to 5pm, through in-person support. The team provides interventions including crisis counselling, short-term intensive case management, support around harm reduction, concurrent disorder, mental health and addictions, health assessment and care as needed by a nurse practitioner and referral to long term primary care

Assessment and case management for seniors (55+), referred due to complex situations including mental health, social isolation, addictions, inability to maintain independent or safe living in the community or inappropriate ED visits. PSW support is available. Service is for approx. 9 months, with warm transfer
