Housing First Intensive Case Management (HFICM)

Housing First Intensive Case Management (HFICM) provides support to individuals in Toronto who are experiencing homelessness, and are living with mental health issues. It is our belief that all people deserve housing, and that adequate housing is a precondition for recovery.

Housing First Intensive Case Management (HFICM)

We collaborate with clients in locating housing, and provide them with the support they need to maintain their housing and achieve their goals. Through regular meetings with a case manager (in our offices or in the community) HFICM provides:

• Help to obtain adequate housing

• Support with making social and community connections that will assist in recovery

• Supportive counseling, physical assessment, and medication management as needed

 Who are HFICM services for?

• Individuals 16 years of age and older

• Individuals who have complex service needs (related to a mental illness and /or substance abuse disorder) and as a result have not been able to access and/or benefit from other programs and services

• Individuals who are homeless or precariously housed (e.g. unsafe, lack of privacy, lack of security) over an extended period of time (typically more than 3 months)

• Individuals must be prepared to meet with program staff on a regular basis until new housing is secure and stable

Where are HFICM services offered?

• HFICM services are offered to individuals who at the time of application are currently homeless or precariously housed within the boundaries of Victoria Park – Hwy 427, Steeles – Eglinton.

• Individuals must be willing to work with a Case Manager to find permanent housing in North York.

• A Referral Form can be obtained by contacting The Access Point at 416-640-1934 or visiting their website at www.theaccesspoint.ca (The Access Point is an Intake Service that provides coordinated access for Case Management Services in Toronto).
