Community Treatment Order Services provide intensive support to individuals who have a mental illness and have been issued a Community Treatment Order to ensure they adhere to their treatment plan.
Community Treatment Order (CTO) Case Management Services
provide intensive case management services to people who are on CTOs. CTO Services work closely with the individual, the hospital-based CTO Coordinators, the individual’s treatment team and other supports to develop a comprehensive Community Treatment Plan (CTP). Assistance is provided to the individual to help meet basic needs, develop skills, access mental and physical health supports, and other community resources. Support in developing the CTO and the CTP is provided by the Centre for Addiction and Mental Health’s (CAMH’s) CTO Coordination Team.
What is a CTO?
• A CTO is an order issued by a physician and agreed to by the individual (and/or their Substitute Decision Maker) which allows the individual to receive care and treatment in their own community, which is less restrictive than being detained in a psychiatric facility.
• A CTO is for those who have had repeated psychiatric admissions and who do not voluntarily engage in outpatient follow-up.

Who are CTO Case Management Services for?
• Individuals who are 16 years of age and older who have been diagnosed with a serious mental illness
• Individuals who are residents of the City of Toronto
• Individuals who are on a CTO where case management services have been identified as part of their Community Treatment Plan
Where are CTO Case Management Services offered?
• CTO Services are offered primarily in the community and/or in clients’ homes.
How do you apply for CTO Case Management Services?
• Referrals can only be accepted through CAMH (Centre for Addiction and Mental Health) CTO Coordinators and Physicians.
