Nurse Practitioner Primary Health Care Clinic
CMHA Toronto recognizes that wellness encompasses both physical and mental health, which is why our Nurse Practitioner Primary Health Care (NP PHC) Clinics provide individuals with mental illness who are not connected to any health care provider with access to primary health care services. Individuals are fully supported by CMHA Toronto support staff and our nurse practitioners, who collaborate closely to provide optimal care and treatment for physical health issues. The NP PHC Clinics aim to remove barriers to primary health care treatment by allowing for flexibility in scheduling, timely appointments and OTN follow-up (when appropriate).
Who are NP Primary Health Care Clinic services for?
NP Primary Health Care Clinic services are for clients currently registered in a CMHA Toronto program, who are NOT connected to a health care provider because the:
- Primary care provider has moved to another community;
- Client has moved to another community;
- Client’s primary care provider is no longer available due to illness/ death/ retirement;
- Client’s primary care provider is no longer available due to change of practice type; OR
- Up until now, the client has not had, or felt they needed, a primary care provider.
What services does the NP PHC Clinic provide?
In the NP Primary Health Care Clinic, our nurse practitioners can perform a complete health history and assessment, manage chronic conditions, perform cancer screenings, perform STI testing, prescribe all medications, order and interpret tests, refer clients to specialists and other health care services, provide immunizations, complete forms (e.g., ODSP, Special diet etc.), prevent health illnesses through screening tests, and provide health promotion, counselling and education.
Connection to the clinic remains on-going until a client decides otherwise. You will continue to receive services from the NP PHC Clinic even after being discharged from other CMHA Toronto programs. Should you move, our nurse practitioners will assist you in finding a new health care provider.
Where are NP PHC Clinic services offered?
CMHA Toronto has two NP Primary Health Care clinics open Monday to Friday from 9:00 a.m. – 3:00 p.m., that are by appointment only. Please note these are not walk-in clinics.

How do you apply to NP PHC Clinic?
To access our services, you must be a CMHA Toronto client. Please inform your case manager that you are interested in receiving primary health care services from a nurse practitioner. Your case manager will refer you to the clinic and schedule an appointment for you.
